ECRI Institute includes inadequate cleaning and disinfection of flexible endoscopes in list


The ECRI Institute includes inadequate cleaning and disinfection of flexible endoscopes in its list of 10 top patient safety hazards of 2016, according to an article on the Becker's Hospital Review website.

ECRI Institute looked into its own database of reported safety events, consulted literature and asked experts for their opinion on what to include on the list.

Other hazards include health IT configurations and organizational workflow that don't support each other.

Failure to embrace a culture of safety also made the list.
