Conn. VA hospital violates OSHA safety guidelines

West Haven facility cited by for exposing employees to 20 on-the-job hazards


The VA Connecticut Healthcare System’s West Haven Campus has been cited by federal workplace safety regulators for exposing employees to 20 on-the-job hazards, including seven that investigators deemed serious, according to an article on the New Haven Register website.

The Occupational Safety and Health Administration found:

• Locked or obstructed emergency exit routes.

• Locked and unmarked emergency exits.

• Lack of guardrails around open pits.

• Absent or inadequate procedures, equipment, training and inspections to prevent unintended activation of machinery during servicing or maintenance.

• Inadequately protected bench grinders.

• Electrical hazards.

OSHA also found floors not maintained in dry and clean condition; unlabeled containers of potentially hazardous chemicals; damaged electrical cords; and failing to de-energize electrical circuits before working on them.
