• A free, downloadable observation form from The World Health Organization that includes a basic compliance calculation table

•  A tool from the The Indiana State Department of Health that includes instructions for the infection prevention staff and observers

•  Two tools from Johns Hopkins Medicine. One includes four rules for conducting hand hygiene observations, and the other tool allows for more details
